WHAT IS PACE?
The Program of All-Inclusive Care for the Elderly
PACE stands for Program of All-Inclusive Care for the Elderly — a Medicare and Medicaid program that combines medical care, home care, transportation, and social services into one coordinated plan, so eligible elderly adults can keep living safely at home instead of moving into a nursing home. An interdisciplinary team of doctors, nurses, therapists, and caregivers manages every part of your care, and many participants also attend a PACE day health center a few times a week for meals, therapy, and social activities as part of their personalized care plan.
