Do I Qualify?

If you meet the following requirements, you are eligible to receive or provide services for Ideal Home Health

To receive care:

  • Are you eligible for Medicaid or think you may be?
  • Do you live in New York?
  • Do you have a condition that requires in home care?

To take care of someone

  • Are you 18 or over?
  • Are you authorized to work in the US?
  • Are you interested in receiving over $19/hr?

Step 1.

Contact us to speak with a compassionate, experienced Ideal Home Health team member to answer all of your registration questions and help you start the process to receive care.

Step 2.

We help you coordinate the details

We guide you through the steps to check Medicaid eligibility, schedule state assessments and coordinate with doctors for paperwork.

Step 3.

Register your friend or family member

We register your eligible friend or family member in our system as your personal care assistant. After this process is complete, they can receive payment for your care.

Step 4.

Receive Care

Your care can begin after a small amount of paperwork. It’s a quick and easy process!
